Each team member must type their own names in the boxes provided below signifying that they agree to the terms specified in the High Adventure Race Liability Form. The Liability form must be also filled out and signed by each team member prior to the start of the race. Teams/Members who fail to turn in a completed form will not be allowed to compete in the race.
